Integrated Service for CWD

Who is your service aimed at?

Any child with a disability 0-18yrs requiring assessment or ongoing support from 2 or more professionals, whose needs are unable to be met through universal services or the SEN process. This includes those children who are potentially going to encounter difficulties due to prematurity, diagnosed conditions or those displaying abnormal neurological behaviour in the neonatal period. Referrals will be considered on an individual basis. If it is decided that a child does not meet the criteria or acceptance onto the pathway would not offer the family anything additional, the referral will be returned to you with an explanation of the rationale.

What does your service offer to schools?

Social work advice and information, response to child care referrals, co-ordinated therapy / social care support to children often delivered in schools.

How do schools access your service?

Instead of completing a number of different referrals to a number of services and agencies, you can now complete just one and send it to the single point of referral (Pathway Coordinator, Rowan House Child Development Unit, Grange Rd South, Hyde, SK14 5NY). The lead professional will ensure that appropriate professionals get involved at the right time for the child and family via the multi-agency allocation panel. A recently completed CAF will also be accepted as a referral. Please consider whether you would be in a position to complete a CAF, as this would greatly help the process of deciding whether a child is accepted onto the pathway.

For serious child care concern, referrals must be made direct to Jubilee Gardens first response officer or the area social work teams.

How do schools contact you?

Where there are child care concerns the referrer must telephone the first response officer who is a social worker at Jubilee Gardens: 0161-371-2060 or to one of the area social work teams.

Alternatively referrals should be made to the single point of referral as above.
